Paciente de 50 años, varón, homosexual con queja de lesión asintomática en labio inferior izquierdo desde hace aproximadamente 2 meses. En la exploración física intraoral se observó lesión ulcerada, con pseudomembrana blanco-amarillenta, no supurativa con bordes endurecidos, elevados y mal delimitados que medía aproximadamente 3 cm en el labio inferior izquierdo. Se realizó una biopsia incisional de la lesión y se le prescribió altas dosis de prednisolona, pero sin éxito. Durante el seguimiento, aparecieron lesiones eritematosas en forma de diana en la región palmar y el informe histopatológico resultante mostró una queilitis liquenoide ulcerada. Los exámenes hematológicos confirmaron la positividad para sífilis en la prueba VDRL. El tratamiento fue de penicilina G benzatina durante 3 semanas.
En el presente caso, la lesión sifilítica tiene características clínicas comunes a otras enfermedades. Por lo tanto, los odontólogos deben conocer sus manifestaciones clínicas, que ayudarían al diagnóstico y tratamiento precoz.

Su evaluación de laboratorio inicial reveló azotemia, niveles elevados de deshidrogenasa láctica, una baja concentración de hemoglobina (4,9 /dL) y un alto recuento de reticulocitos corregidos (4,7%) con pruebas de Coombs directas e indirectas negativas. El paciente fue trasladado a la UCI, donde recibió terapia de apoyo con hemodiálisis, ventilación mecánica, fármacos vasoactivos y transfusiones de concentrados de hematíes. La evolución después de 1 mes fue favorable y fue dado de alta sin anemia y con función renal normal. Tres meses después del alta, el estudio de detección de glucosa-6-fosfato-deshidrogenasa no demostró actividad enzimática detectable.Palabras clave
